Employees concerned about receiving a COVID-19 vaccine due to a disability or medical condition should consult their medical provider. If, after consulting a medical provider, it is determined that an accommodation is needed, the employee should complete the DRC COVID-19 Vaccine Accommodation Request form. Additionally, the employee’s medical provider will need to complete the COVID-19 Vaccine Medical Provider Form.
It is the employee’s responsibility to ensure the DRC receives requested documentation. If you do not receive confirmation from the DRC that we have received your documentation within three business days, please contact us at DRC-Covid19Vax@email.arizona.edu. Once received, any medical information will be maintained at the DRC and kept confidential.
The DRC will review the submitted information to determine eligibility for accommodation.
